This program was comprised of two components: introduction of social services to the case management system “Νοιάζομαι” – “I care” and the pilot transformation of one Health Clinic to a Multiservice Municipal Center .

1.

Helped rationalize the fragmented and dispersed system through which social services were delivered.

2.

The introduction of a Case Management System aimed to improve the level and quality of support for the thousands of Athenians in need of social services.

3.

The overall objective of the pilot operation of a Multiservice Center was to test the methodology for the decentralized integrated approach in delivering social and health services by the City of Athens and to assist vulnerable groups re-integrate into society.
